Question

l Client appeared to be in an anxious state evidenced by client crying linked with client's rapid speech and thought process. Client informed BHS of feeling as if her needs and situation were not taken seriously due to it being almost a year and she yet obtained stable housing. BHS ensured the client that her needs and situation are taken seriously and apologized for the unpredictable wait-period that the client is subjected to. Client requested to complete the session outside due to needing fresh air. Once outside , the client engaged in deep breathing and positive self-talk. please complete the progress note.
