You are the credentialing manager for a medium-sized hospital (80-120 beds).
Part 1: Provide a summary description and Web link of where you can find credentialing (licensure or certification) information for physicians, nurses, and other nonphysician providers in your state.
Part 2: List the academic qualifications and continuing education requirements for licensure or certification for at least 3 provider types, as specified by your state's Department of Health Web site. The following are some examples that can be used:
