As we explores the creation and maintenance of high-performing organizations. As we see, there are a number of contributing factors like (learning the organization, team work and empowerment, ethics/ethical behavior, job satisfaction and employee engagement, brand alignment) towards establishing what we call "high-performing organizations".

What are two top reasons you have seen in your own experience that have prohibited an organization (or team) from being a high-performer? Be sure to describe the organization/team and articulate what those two reasons are with examples. Be sure to defend why you believe those two reasons inhibited performance. In addition, outline how HR could have done to help improve performance.
