Within the state of Ohio, approximately 48% of adults report at least one chronic health condition with that number rising to approximately 76% in adults 65 and older. Chronic diseases shorten lives, reduce quality of life, and create considerable burden for caregivers.

Discussion Question: Imagine that you are a public health professional working for the state of Ohio who has been tasked with creating a plan to combat the burden of chronic diseases within the state.
