Background: Within the United States 6 out of 10 American's are living with a chronic disease. Within the state of Ohio, approximately 48% of adults report at least one chronic health condition with that number rising to approximately 76% in adults 65 and older. Chronic diseases shorten lives, reduce quality of life, and create considerable burden for caregivers.

Discussion Question: Imagine that you are a public health professional working for the state of Ohio who has been tasked with creating a plan to combat the burden of chronic diseases within the state.

Consider including some of the following concepts

Intervention(s) aimed at prevention and management of one or more of these chronic health conditions
