Assessment steps:
1. PROGRAM PLANNING LOGIC MODEL
Using one of the logic models discussed in the unit materials, create your own logic model for the Health Promotion Plan that you developed in Assessment 2.
Your model should include all of the common components of logic models:
- Goal
- Inputs
- Activities
- Audience
- Outputs
- Outcomes
You should also include any additional components that are relevant to your health issue, for example:
- Situation
- Assumptions
- External factors
- Strategies
2. EVALUATION
Using the evaluation logic model template, create a structured and systematic evaluation logic model to evaluate your program/campaign.
Using the evaluation logic model you have created, develop a comprehensive evaluation plan for your program. Describe how you will evaluate your program, and the measures you will use to determine whether your goals and objectives have been met. Remember, there are different types of evaluation needed at different stages. For this assignment, you need to consider process, impact,and outcome
