Problem: Create an SBAR after taking care of a 68year old female on full code admitted for mesenteric ischemia on a consistent carbohydrate diet. she had bowel resection surgery , has a wound vac, ostomy bag, wounds appear to be healing normally. She complains of severe abdominal pain radiating to her chest. Has edema on both legs. Patient had TPN which was discontinued. Patient is on antibiotics. Patient is alert and oriented, had BM in ostomy bag and straw yellow urine. Patient's breath sounds were auscultated and diminished bilaterally. Other medical conditions are obesity, HTN, diabetes mellitus. s1 s2 sounds were heard and abdomen was tender to touch with no distension.
