Task description

As a Group/Team, you are required to consider the Patient Case Study provided.

You will research and identify any actual/potential medication issues and create an interprofessional plan to solve these issues using a selected framework that assesses the safe, effective, appropriate, and judicious treatment within a comprehensive approach of the patient case to provide an action plan. This assists you to promote Quality Use of Medicines (QUM) in the context of patient care, both as an individual health practitioner, and as part of an interprofessional healthcare team.

Patient Assessment

• Your A2 Canvas Group's submission MUST use the Assessment 2 Patient Assessment Template provided.

• Your Group's submission must comply with the following specifications:

• Maximum of SIX (6) pages or less

• Word Count minimum of 1700 - maximum of 1800 words.

• Page 1: Cover Sheet (excluded from word count)

• Pages 2-5: the body of your written assignment with QUT citewrite Vancouver in-text citations.
