The decisions made by senior Leadership and the organizational culture fostered an environment that proved to be disastrous leading to fatality and mass destruction within the surrounding community as a result of negligence both at the senior and supervisory levels. What are some of the challenges to changing norms, values, and beliefs in a large organization such as BP? Several attempts were made over the years to establish more structure and to create an environment that fostered transparency emphasizing communication across the organization.
