Scenario/Summary

In this Lab, you will create an entity relationship diagram for a portion of a database that a medical laboratory might use to keep track of physician orders for blood tests.

The major entities involved in this database design are the following.
– Physicians
– Patients
– Test panels
– Tests

These are the business rules that apply to these entities.
– Each physician orders zero to many test panels; each test panel is ordered by one and only one physician.
– Each test panel is ordered for one and only one patient; each patient is associated with zero to many test panels.
– Each test panel includes one to many tests; each test is included in zero to many test panels.

Deliverables
Submit a single Microsoft Visio. This file will contain two diagram pages and/or a MS Word file.

VSDX file includes both diagrams.

1. Conceptual: A conceptual data model diagram of this database - Vsdx file or Screen print in MS Word

2. Logical: A logical data model diagram of this database 0 Vsdx file or Screen Print in Word.

Document file with proposed data types for each table in the vsdx file. (NOTE: MS Word document can contain two screen prints of diagrams listed above.)
