Portfolio activity
In Assessment 1 you will be locating, accessing and appraising information about your profession's standards and guidelines relating the attributes of professionalism. This modules Portfolio activity will help you prepare for your first assessment.
As part of your portfolio create an annotated bibliography of resources you can use to maintain an up-to-date awareness of these standards and guidelines.
Include resources from government, peak professional bodies relevant to your practice - these may be professional societies, communities or practice, field of practice organisations or clearing houses for evidence-based practice resources in your area.
There are also three readings for this week - two come from the Australian Council of the Professions and one is about professional identity. These give you more information about what makes a profession, why it is important and why identifying with a profession is so influential in practice behaviours.
Create an annotated bibliography of 3 documents.
The SDL activity for Week 2 requires development of a learning plan.
Using the 1-ISCS01 CPD template shared in this modules tutorial, complete a learning plan .
If your profession (or target profession) is registered with AHPRA, you can create a version that uses the recommended CPD portfolio template for that profession (please FIRST advise your tutor in advance if this is what you will be doing)
If your profession is accredited but not registered, and the peak body has a recommended CPD portfolio template, please discuss with your tutor beforehand to see whether or not the template meets content requirements for this SDL activity. If it does you can use that template instead of the HSC501 Module 2 CPD template.

Portfolio resources and reflections this module explore the material on types of teams, attributes for successful team functioning, and the interaction of personal attributes, professional identity and roles within teams.
This SDL this week will support your understanding of the principles of interdisciplinary team work.

This portfolio activity will support your development and implementation of interprofessional practice competencies to achieve collaborative results. Complete the Self-Directed Learning activity from Module 4. Upload the document you create to your portfolio.

Annotated Bibliography
A bibliography is a list of texts or other sources of information (web pages, reports, books (chapters), images) that have been reviewed while researching or exploring an issue either for research, for an assignment or for learning purposes. (Purdue University, 2024)
An annotated bibliography is where your include a notation or summary of those sources of information.
There are different styles to annotations that you can consider.
• Summarising / Descriptive
• Assessing / Evaluative
• Reflecting (Massasoit Community College, 2017; Purdue University n.d.)
You may choose one or more of these for your annotation. In the case of completing an annotated bibliography for an assessment task (not part of this unit) you will be given explicit instruction on the style, length and format etc
A quick summary of each is below.
• Summarise: Consider outlining the main points or arguments? You could think about writing your summary in away that is you explaining the source to another student. In the case of an article what is the hypothesis or research aim/ purpose. What was the research methodology? Any conclusions?
• Assess: once you have summarised your source, it is time to evaluate it. Is it useful ? If your are completing an annotated bibliography on a topic with a number of articles what are the links or differences with other sources. Is the source a quality source? What is the goal of this document how might that impact on the information in it or how it is presented? Is the methodology sound? are there
any strengths or weaknesses?
• Reflect: Now its time see how your reading of this resource fits into your research! learning. Was it helpful? Does shape an argument you want to put forward? How can you use this source in your learning/ research/ assignments? Have you changed changed how you think about the topic/ issue?
Then watch this example of a training video below, promoting effective teamwork (3:53 min).
Reflect on the the necessary skills identified here for collaborative teamwork. Compare and contrast in a table the skills for collaborative teamwork that may result in benefits for improved patient care to those solutions that Anna puts forward. This exercise will form your portfolio piece for Week 4. Try to reflect from the different perspectives - a person's view of coordination of her health and social care compared to skills of collaborative functioning of a team to deliver improved health outcomes.
First watch this video of Anna Severwright, the Co-chair of the Coalition for Collaborative Care (C4CC) in UK talking about keeping the person at the centre of collaborative working between health and social care.
Note 3-4 issues that she describes with her multiple health and social care teams. She also presents what she feels are some person-centred solutions to improve things in her care and her life.
