Patient Name: Eldon Drake

HISTORY: Mr. Drake is an 85-year-old Caucasian male who was admitted to the hospital on April 29, 2013, for fever and confusion. He developed expressive aphasia while in the hospital. His history is positive for leg pain, paresthesia, lumbar spinal stenosis and radiculopathy of the S1 nerve root.

OBJECTIVE FINDINGS: On physical examination, the patient was alert but disoriented to time and place. Blood pressure was 160/80, Pulse 96, Respirations 20, Temperature 103.8 F. No focal neurologic deficits. Chest x-ray, urinalysis, and blood cultures were negative. A neurology consultation was obtained. MRI of the brain was performed, which demonstrated encephalitis. An EEG was markedly abnormal for his age.

TREATMENT SUMMARY: After 10 days of IV acyclovir, the patient slowly began to improve and began to talk normally. He was then discharged from the hospital.
