BACKGROUND: There was an incident at Penmen General Hospital (PGH) involving a nurse who administered the wrong medication that led to a patient death. Upon further exploration into the incident, it was found that the nurse who administered the medication had accessed the medication dispensing unit and selected "Override" to obtain the medication. The Chief Nursing Officer has been working with the compliance team and nursing leadership to create a quality improvement initiative for overrides. There has been much concern about this outcome related to patient safety, quality improvement, and risks to nursing practice due to systems and human errors.
