Background Information

The Centers for Medicare and Medicaid (CMS) set a target percentage readmission rate for various types of patients, including Total Joint Replacement. The target rate for Total Joint Replacement is 4% for the current year.

The Problem

The readmission rate for Total Joint Replacement at Global Memorial Hospital is 10.42%, with 50 out of 480 Total Joint Replacement patients being readmitted within the past six months. This rate is 6.42% higher than the CMS target and will result in the hospital paying the penalty to CMS. The breakdown of cases by the surgeon is as follows:

Dr. Chill - 41 cases

Dr. Hugs - 146 cases

Dr. Wagoner - 105 cases

Dr. Franks - 188 cases

Total = 480 Cases

The Service Line Director of Orthopaedics has contacted the Director of Health Information Management for assistance.

The Project

The Director of Health Information Management had a coder review each chart to determine the reason for readmission.
