Steve Coff has had an upper respiratory tract infection for several days. He was very busy so did not take any time off work and continued to push through. He has been controlling his symptoms with over the counter cold medications.
He is using Benadryl cough medicine and Tylenol for a low grade fever of 38.2 oral and says he is experiencing chills and sweats and general achiness. He states "I feel really cold, then I feel like I am burning up" and "I was coughing so much it made me throw up". He is 69 years old, smoke s pack of cigarettes a day, is 5'11" tall and weighs 130 kg. He says he feels he has lost weight because he hasn't eaten and did not drink anything yet today as he feels too ill and has no appetite.
Last night he began coughing up thick green sputum and says he feels breathless if he exerts himself at all. He was sent for a chest x-ray hand was diagnosed with a left lower lobe pneumonia. Steve has a history of hypertension and high cholesterol. He does not get paid if he misses work and is concerned because he still has young children at home. His sister is staying with the kids as his wife is back in the Philippines visiting her aging parents.
His admission vitals at 1000 are as follows:
BP 148/92, respirations 28 /minute, heart rate 102 / minute, temp. 38.8 oral and oxygen saturation 90% at rest on room air
