You are required to create a dynamic web-based application in PHP, SQL and JavaScript and this is an individual project.

Project Requirements

1. The application should have least at 5 web pages linked to a navigation bar.

2. You should have a landing page or home page

3. Use an external cascading style sheet to style your web application

4. All the 5 web pages in your application should be created in PHP

5. The web application should have separate pages for data entry and data output

6. For data entry, use web forms and ensure that all the captured data is stored in a database using SQL. The web form should have 5 fields.

7. Validate fields in your web forms using JavaScript to prevent users from submitting blank forms to database

8. Use SQL to retrieve your data from the database display well formatted web reports.
