You have now reached the final stage of case management! You will create a discharge plan which summarizes the outcome of the work completed with the mock client. Refer back to the service plan; were goals completed? Were referrals accepted? Did the client follow through on their tasks? Have they been put on waitlist? What new services is the client now connected to? In 1-2 pages summarize the end results of the work with the client.

Considerations for Discharge Plan:

· Reason for initial referral

· Presenting needs

· Services provided

· Any unusual occurrences

· Continued circle of care (ex. Family members, new service and/or health providers)

· Outcome of goals/new insights on health status/change in needs

· Consider any potential problems that could be anticipated post discharge (e.g., lack of consistent informal support, home situation, cultural differences, lack of resources, physical restrictions, challenges with motivation, history of noncompliance) and how they can be addressed.
