Creating a "Culture of Safety"

  • A leadership-driven organizational "culture of safety" is comprised of, but not limited to, the following essential elements:
  1. Recognition that we "live in a high-risk environment" and that errors can happen to anyone at any time;
  2. A commitment to the fact that Patient Safety is everyone's job - every minute, every day;
  3. A "Just Culture" of transparent and open reporting of errors and near misses; and
  4. Key leadership participation and support.

The "culture of safety" does not happen by accident!

Question 1: There are well-researched actions that leadership can take to create such a culture. Based on your readings and research, what do you think are the three most important actions that you, as a leader, could can take to create "a culture of safety' in your organization ?
