Per patient, wound bled minimally, and patient performed no cleaning of wound. Redness developed yesterday, and patient sought treatment this morning when mild swelling, discomfort, and low-grade fever developed. On the basis of reported history, do not suspect exposure to methicillin-resistant Staphylococcus aureus. Suspected organism: Staphylococcus aureus. Labs indicate increased white blood cell count with leftward shift. Wound culture deferred at this time because skin at puncture site is fully intact. Blood for cultures drawn in emergency department; results pending. Plan for IV nafcillin, topical puncture site cleaning, dry gauze dressing, and monitoring. Regular diet and activity. Plan to evaluate on Tues AM for probable discharge.
