Problem

Create a CERT-type review of your practice's administration of vitamin B-12 injections. Using the 2013 Noridian Administrative Services, the MAC for jurisdictions E and F, Local Coverage Determination (L33502) as a guide, describe how you would create a RAC-type query based upon current CPT, HCPCS, and ICD-10-CM codes to check to see how your practice is doing complying with the LCD for your 15-physician internal medicine group. Note that the 2013 LCD uses the old ICD-9 diagnosis codes, so you will need to crosswalk them to ICD-10 diagnosis codes. For example, 162.9 was the ICD-9 diagnosis code for lung cancer NOS. You will need to find the current ICD-10 code by going to the Table of Neoplasms and finding the code from the alpha description. You will do the same for all of the other diagnoses listed in the LCD. Pernicious anemia would be found in the ICD-10 alphabetic index under the main term anemia and the subterm pernicious.
