a. History of the patient's problems
b. Physical findings
c. Psychiatric findings (Using DSM5-TR diagnosis only.)

6. Recommendations

a. Treatment recommendations according to the US clinical guidelines.
b. Patient education for management and anticipatory guidance.
c. Non-pharmaceutical, cultural, and spiritual considerations must be addressed.
