Spouse brought client into ED after finding a suicide note and the gun cabinet unlocked. Reports that the client is "not the same person" after being hospitalized 3 months ago with severe COVID-19 and mechanically ventilated for over 2 weeks. Client states that since being discharged from the hospital, has periods of heart palpitations, insomnia, apathy, depressed mood, and anorexia. Reports that drinking alcohol every night helps to relax, but still becomes anxious at times. Is very worried about getting COVID again because the client's sibling, whom recently visited, tested positive yesterday. Has frequent nightmares about the hospital experience because client nearly died. Lost 35 pounds (15.9 kg) during hospital stay but states weighs more than 260 pounds (117.9 kg). Was considering suicide this evening because the client was afraid of possibly "being reinfected and dying this time." Recently diagnosed with type 2 DM, controlled with diet and metformin.
