A container vessel was passing off the coast of Portugal on its way to the Suez Canal. Two crew members. John and Albert were walking along the weather deck of the vessel. Suddenly John slipped and fell, and a liquid on the deck impregnated his hands, arms, T- Shirt and jean. A leakage of a yellow liquid was noticed form an adjacent container. With Albert’s assistance John was soon back on his feet, and they went to the chief officer and reported the incident. Shortly after John had changed his clothes and washed he complained about headache, stomach pains and weakness, which Albert reported to the chief officer.

The Captain was informed of the accident, the leaking container was found, and when the stowage position and container number was known it could be determined that it contained two different liquids, classified as dangerous goods:

1. five steel drums ethyl chloroformate

2. three plastic drums UN 3018
