Congestive Heart Failure (CHF)

You will be assigned one of the above topics. You will need to write a care plan using the following information as a summary of the disease, making sure to include all of the points below and then teach this topic to a classmate. You will use this care plan to teach a classmate and you will be critiqued by your classmate:

1. Pathophysiology of the disease: In your own words, write the pathophysiology of the disorder and relate the clinical manifestations of each back to the pathophysiology based on the information you taught your peer.

2. Clinical manifestations of the disease (at least two assessment findings)

3. Two nursing diagnoses: Discuss 2 nursing diagnoses common to patients and the interventions you chose for each diagnosis.

4. Treatment plan (at least two interventions: interprofessional and nursing care) and the rationale for interventions

5. Four patient teaching points for this disease: Include the disease, the treatment plan, and the evaluation of the effectiveness of your teaching.
