Question - Tony, age 34 has a history of many colds.   He says he often gets a cold in the fall and it stays until spring. He smokes ½ pack cigarettes a day.  He has come to the clinic because his sputum has increased in amount and is now thick and greenish in colour.  You note that his vital signs are as follows:  130/78, 38°-80 - 22.   He looks flushed in the face, capillary refill is 1 second, but the nail beds look bluish.  His oxygen saturation is 90%.  When you listen to his chest, you hear gurgles in the lower right lobe.

1. Conduct an assessment to collect pertinent data.
