Clinical Case Study Assignment

Overview
This assignment requires you to conduct a case study for a patient who attends the pharmacy or is a patient identified from the site where you do your placement and has some sort of presenting issue/complaint. Choose a patient who is taking at least FIVE different medications.

The case study should follow a similar format to the case studies in the tutorials and cover the following:

Part 1:
• Details of the presenting complaint and its history
• Information gathered about the patient including past medical history, family and social history, medication history, and relevant observations about the patient.

Part 2:

• Summary of your assessment of the MAIN clinical issue(s)
• A proposed management strategy including monitoring
