Question: In class we discussed the concept of Root Cause Analysis (RCA) in relation to Japanese Firms. RCA is based on the belief that problems are best solved by attempting to correct or eliminate root causes, as opposed to merely addressing the immediately obvious symptoms.

In doing so, we learned a casual factor identification tool known as "5 whys" - Ask "why" until you get to the root of the problem. Please use the "5 whys" and drill down (break down) the following problem into small, detailed parts to better understand the big picture, and finally recommend one major possible solution.
