Today's concept of "medical necessity" determines the extent to which individuals with health conditions receive health care services. "Medical necessity" is the measure of whether a procedure is appropriate for the diagnosis and the treatment of a condition. This decision making process is based on the insurance company and the provider's documentation. Rhinoplasty is sometimes considered cosmetic and not covered by insurance.
