HISTORY: A 29-year-old married computer programmer is complaining of "something strange" going on in his scrotum. Last month while doing his testicular self-examination felt a lump in his left testis. He waited a month and felt the area again, but the lump was still there. Has some "aching" in his left testis but denies any pain with urination or sexual intercourse.Denies any fever, malaise, or night sweats. Past Medical History: Surgeries: Groin surgery when he was a baby and a tonsillectomy as a teenager. Family Hx: Both parents are "healthy." Social Hx: denies tobacco or illegal drugs. Drinks alcohol occasionally. He eats a healthy diet and works out at the gym five times a week.

PHYSICAL EXAM: General Survey: muscular, healthy, appears younger than stated age. Vital Signs: Temp 98.4, HR 72, BP 118/72 GU: Penis is circumcised with no lesions; scar in his right inguinal region. No lymphadenopathy. Scrotum has a large, palpable mass on the left. (Placing a finger through the inguinal ring on the right, you have the patient bear down. Nothing is felt. You attempt to place your finger through the left inguinal ring but cannot get above the mass.) Rectal exam: prostate nonenlarged with medial.
