Program Proposal Assignment
Assignment Overview: For this assignment, you will assume that you are employed by the local Health Department as a public health professional (any one of the five local counties in northern VA). You have been approached by a funder, called the Champion for a Healthy City Foundation, to submit a proposal for a grant. The Foundation is excited to fund programs that address specific, identified health needs within the local community. The Health Department recently compiled a summary of key health indicators that focus on health behaviors, morbidity and mortality. The information can be found on the community health dashboard. The Foundation is soliciting proposals for programs that will focus on improving the health of the community using the information on the website. You will use the Generalized Model for the planning of your program.
The Foundation has shared that grants will be awarded in the amount of up to $100,000 for a 12-month program. Your Program Proposal will be broken down into four (4) written parts plus a presentation:
Program Proposal Part 1 will include a title, at least 5 references and a 2-3 page rationale that includes relevant and specific data and background on the health issue and community chosen, a health behavior theory or model and at least one evidence-based strategy.
The Program Proposal Part 2 will have a goal statement, 2 SMART process objectives, 2 SMART impact objectives, a 4-5 page narrative, a Gantt chart (or timeline) and a completed logic model. The narrative includes a detailed description of the proposed program activities.
The Program Proposal Part 3 includes a detailed budget (a template will be provided), a 1-2 page evaluation plan and a description of at least 2 deliverables (marketing tool, assessment tool, lesson plan, developed curriculum, etc.).
The Program Proposal Part 4 includes finalized (edited and corrected) Program Proposal Parts 1 -3, any appendices as appropriate and at least 10 APA formatted references. The final Program Proposal will not be longer than 15 pages, not including references and appendices.
The Presentation will be 5-7 minutes long. You will prepare and deliver a power point presentation to the class. Part of your grade for the presentation will be two (2) peer reviews of 2 of your classmates' presentations.

• Your first step is to review the website (community health dashboard for your chosen county) and select a health topic. Everyone must pick a health topic identified on the site.
• After you identify a specific health topic (or problem), review the Community Guide website (or similar) to identify an evidence-based strategy or intervention. This will be the focus of your proposal.
• Find at least 5 references that you will use in developing your rationale. You will need to list these 5 references in correct APA format. Your references will be reviewed for the variety of sources you are using, the titles of the sources you are using and the relevancy of the information to your topic.
• Write a 2-3 page rationale. This will include background and data on the health topic, a description of the community of interest, why the health topic is a problem, how you propose to address the health topic and expected outcomes/benefits.

• Write two (2) process and two (2) impact SMART objectives for your proposed program.
• Write a 4-5 page narrative to include details about your proposed program. Include a paragraph on how the activities are related to the health behavior theory or model you chose. Include the why, how, what and when of your proposed program activities. BE SPECIFIC!
• Complete a Gantt chart (or timeline) on how your proposed program will be implemented over the 12 month period. A template will be provided. This can be an appendix.
• Complete a logic model for your proposed program. A template will be provided. This can be an appendix.
• Include Part 1 and your growing list of references.

• Complete the budget template for your proposed program. You cannot exceed $100,000 in your request but you can include up to $10,000 in-kind. BE SPECIFIC! If you are buying 100 binders, list the source and cost of each binder. Include budget narratives in the spaces provided in the template. Include fringe benefts and indirect costs as appropriate. This can be an appendix.
• Write a 1-2 page evaluation plan. How will your objectives be measured? What data will you collect? How will you know if you were successful? Who will conduct the evaluation? When will it be done?
• List and describe at least 2 deliverables. These are physical items you can shows the funder and the community as products of the proposed program. This might be an evaluation tool, a curriculum, a website, an app or a marketing piece.
• Include Parts 1 and 2 and your growing list of references.
