M. T. is a 62-year-old male who presents to the office with complaints of "intermittent episodes of lightheadedness for the past several months with fatigue". He has a history of Hypertension, was prescribed Metoprolol 25 mg a day 6 months ago. He denies chest pain and shortness of breath. He notices a fluttering sensation when he has these episodes and feels like his heart is racing, his blood pressure is 120/80 mm Hg, his heart rate is 140 bpm and irregular, lung fields are clear to auscultation, capillary refill is < 3 seconds, states gets occasional headaches with the light headedness, feels less energetic than usual, notes has been under a lot of stress at work lately and has not been his antihypertensives every day. An EKG performed in the office confirms atrial fibrillation.. First, identify correlating Subjective and Objective data by placing into the appropriate column in the chart. Then, discuss any additional questions you would ask as part of the History of Presenting Illness (HPI) and Subjective Review of Systems (ROS).
