Question

Garrett, age 72, comes to see you for complaints of depressed mood, constant fatigue, poor appetite and vague aches and pains. Garrett is accompanied by his son. The son does the majority of talking during the interview while Garrett sits quietly, a worried expression on his face. Garrett's son states that for the past two months, Garrett has not been eating well, has lost approximately 10 pounds, and has difficulties falling and staying asleep. Garrett used to be a very active and sociable man who enjoyed golfing, going on outings with his children and grandchildren and playing cards with friends. His son states that now Garrett refuses to leave the home or participate in any activities due to being too tired and not feeling well. Six months ago, Garrett lost his wife of 45 years to breast cancer. Since then, he has been increasingly isolated. He frequently expresses passive suicidal ideation, making statements such as: "You would all be better off without me." Psychiatric History: There no history of inpatient or outpatient psychiatric treatment. He has no history of suicidal and/or self- harming behaviors. Garrett's son states that Garrett was depressed when his wife was first diagnosed with cancer.
