Question

A 44-year-old female presents to the family practice clinic with a chief complaint of generalized abdominal pain and diarrhea x 3 months that has been coming and going. The patient also noted that she had decreased appetite on occasion. Physical examination, including abdominal exam, was within normal limits. The NP ordered baseline labs including CBC, CMP, and lipase as well as a KUB x-ray. All were normal. The patient was discharged home from the clinic with a prescription for dicyclomine 20 mg QID PRN. The NP noted that she had a high suspicion of irritable bowel syndrome. The patient was seen 3 additional times following that with similar complaints and requested dicyclomine refills. The NP was not aware of the patient's family history of colon cancer. Eight months following the initial visit for abdominal pain, she developed severe abdominal pain. She went to the ED where the CT scan showed stage 4 colon cancer and a bowel obstruction. Chemotherapy was initiated, but the patient died 12 months later. The NP was sued for malpractice by the patient's family.
