Assignment: Clinical Monitor Report

Introduction

When participating in an external sponsor's clinical trial, the sponsor will be on-site to monitor data, or will electronically monitor data that is collected to assure accurateness of the data. In this assignment, you will be the Clinical Trial Monitor that is coming to audit the data collected for the patients enrolled in the Stroke Study 1 research study.

Instructions

1. Use the Medical Record Source document to verify that the information in the Stroke Study 1 is correct. Remember, the Medical Record is the main source of information that is transferred to the CRF. Therefore, it is important to make sure that the data is transferred accurately.

2. Compare the Medical Record Source with the Stroke Study 1 CRF . Verify that the data that was collected was transferred correctly to the CRF.

3. When errors are found, they must be reported using the Sample Monitor Letter . Complete the Sample Monitor Letter with the errors.
