The community assessment action plan will basically set the vision by answering questions such as what actions need to take place to reach certain objectives, what direction are they going, and why it is important. It also sets the mission by stating the purpose, principles, and the direction the assessment is going. The action plan states the goals and objectives, and also describes the activities and the desired outcomes.
The first steps I would do in designing an action plan would be to analyze the results of the collected data and then choose team to work together to help develop an action plan for each strategy. The teams would consist of members of the community such as community leaders, members of the medical community, teachers, local business owners, and parents. They all will be giving task based off their skills sets, resources, and unique assets. Each strategy would have its own set of goals and objectives, a list of what the community's needs are, and the asset available to reach them. The key is to choose goals that supports the outcomes we are trying to reach. There would also be listed of the objectives, their anticipated impact, and a list of responsibilities of each objective. There would be a timeline for conducting strategies so that everyone would have an idea of when everything should be completed. The plan itself would be flexible so that it could account of challenges or obstacles that many occur.
I believe the best way to put my plan into action is to stagger the implementation of each strategy in segments. This way they could each be monitored and measured. Each strategy should also be introduced to the community by divisions so that adjustments could be made if need be.
