Case study

Mark, aged 22, admitted to hospital with the complaints of acute exacerbation of asthma. Mark has a past medical history of Asthma, recurrent lower respiratory tract infection and testicular torsion which was surgically corrected at the age of 11. Mark has been diagnosed with Asthma since he turned 16. Mark was on a preventer inhaler (Symbicort) and Ventolin to be taken when he has asthma attacks. Mark commenced smoking 2 years ago due to peer pressure. Mark is alert and oriented with GCS 15/15. He is haemodynamically stable. He lives with his parents and is currently enrolled into TAFE for bricklaying course. His classes were missed multiple times due to respiratory tract infections and asthma exacerbations.

1. What is asthma? What are the most common signs and symptoms of asthma?

2. Describe the impact of this chronic disease on Mark as an individual. What limitations does Mark face due to this disease?

3. Based on evidence-based practice, develop a nursing care plan for the client.

4. Mark is for discharge from hospital today. Physician has commenced him on a new inhaler. Mark has to be enabled to self-manage his health care needs in the community. Describe the plan of care/ strategies that you would implement to achieve the above. (Hint: Explain nursing actions in hospital, follow ups and community referrals that you would carry out for this client)
