Question

This is a 58 y/o male who presents to the primary care clinic with complaints of abdominal pain. The pain is described as a gnawing sensation, 6/10, mostly in the epigastric region. It is worse on an empty stomach and after consumption of anything acidic, including soda/pop. It started about a week ago, and seems to wax and wane. He denies nausea, vomiting, or diarrhea. Stools seem to be a bit darker than usual. Past medical history: osteoarthritis of the right knee, diet-controlled hypertension Medications: meloxicam once daily for arthritis pain, diclofenac gel topically as needed Physical exam: BP 130/80, HR 78, SP02 97% on room air, BMI 28. Mild epigastric tenderness to palpation. Abdomen is soft without rebound tenderness. Guaiac stool positive. Rectal exam otherwise normal.
