Question

A 43-year-old client who is pregnant and homeless is brought to the ED by EMS when the client was found lying in a puddle of blood this morning at the homeless shelter. The client reports being seen at the mobile health care clinic for the homeless when becoming pregnant about eight months ago. The client reports taking prenatal vitamins for one month but never returned to the mobile health care clinic for follow-up prenatal care. The nurse performs an assessment on the client and documents the following. Nurse's notes: 0750: This 43-year-old client is approximately 32 weeks' gestation. The client reports this is the second pregnancy; the client had a miscarriage at 10 weeks' gestation with the first pregnancy. The client reports no medical history and denies the use of substances, although reports a history of smoking cigarettes when "I could get my hands on some," but stopped when finding out about the pregnancy. The client states being homeless for about two years, ever since the COVID-19 pandemic began with the loss of so many jobs. The client states that the other parent is also homeless and has disappeared when finding out about the pregnancy. The client has no family.
