The emergency department nurse cares for a 43-year-old female client presenting with vomiting and severe abdominal pain. The nurse completes triage and documents a note. 43-year-old female client presents to the emergency department (ED), arriving bent over and holding right side. The client reports "severe abdominal pain" rated 10/10 that is the worst when trying to urinate and feels like the last time the client had a kidney stone. The pain is constant. Vomiting × 2. Respirations shallow. Client is alert and oriented, lying on right side for comfort. Denies chest pain or any recent changes in bowel and bladder function. 18-gauge saline lock needle placed to left antecubital vein, laboratory specimens obtained and sent, continuous blood pressure and pulse oximetry connected. Blood pressure: 175/95 mm Hg Heart rate: 102 beats/min Respiratory rate: 28 breaths/min Temperature: 98.2°F (36.7°C) Oxygen saturation: 97% on room air Medical/surgical history includes tonsils/adenoidectomy, appendectomy, cholecystectomy, kidney stones, total abdominal hysterectomy with oopherectomy, diverticulitis, endometriosis, depression, hypothyroidism, morbid obesity.
