Question

It is important for a nurse to establish a baseline of vital signs to get an overview of the client's current health status or normal health status. These vital sign measurements can help to identify problems, develop a plan of care for the patient, and monitor the response to interventions or treatments. For example, a client presents to the emergency room with an elevated blood pressure, elevated pulse, and shortness of breath. While assessing the patient, the nurse can ask if they know their typical vital signs. Perhaps, this patient has a cardiac condition and their blood pressure is typically elevated. By establishing a baseline, when interventions or treatments are given to the patient, the nurse can monitor the patient and assess the effectiveness.
