Question

History and Assessment 12/12/XX 18-month-old female is admitted to the pediatric unit for chronic vomiting and diarrhea with documented weight loss. Lives at home with parents. No sick contacts. Myringotomy done at 12 months old for recurrent ear infections. No other past medical or surgical history. Immunizations up to date. Playing in exam room. PERRL, moves all extremities well. Mucous membranes moist and pale. Lungs clear to auscultation. Abdominal bloating noted. Per parents, foul-smelling, nonbloody diarrhea occurs once to twice a day and vomiting once a day. Increasing flatulence. Generalized pallor noted.
