Mr. Jones, is an 80-year-old patient you have been following in your primary care clinic for ten years for his chronic renal disease. His oncologist had given him a prescription for oxycodone/acetaminophen (Percocet®) to take as needed for cancer-related pain. He is currently taking two oxycodone/acetaminophen 5/325 mg tablets every three hours for bone pain related to lung cancer, metastatic to the liver, bone and brain. His pain is a constant "6" on a 0 to 10 scale and is limiting his ability to join his buddies in his assisted living facility's community card games. You should: Contact his oncologist and collaboratively develop a plan for improving his pain regimen. Recommend he continue to take the pain medication ordered by his oncologist, but limit it to every four hours. Switch him from the current pain medication to long acting oxycodone (OxyContin) 10 mg twice a day. Add an NSAID to his current pain regimen to better manage his bone pain.
