Question

Harvey is a 48-year-old male construction worker who struggles with chronic lower back pain that radiates into his right leg. Harvey arrives at the clinic today and the nurse performs his initial assessment. He grimaces occasionally. He states his pain is 7 out of 10 on a 0 to 10 pain scale that can be sharp and tingly. He shares that the pain is worse when standing, laying down or transitioning between positions. His vital signs are: heart rate 99, respirations 28, temperature 98.6 and blood pressure 144/88. Circulatory, sensory, motor is intact X 4 extremities. The nurse reviews documentation from the EHR and notes that Harvey is in a pain management program. Harvey states he would like to have more comfort and improved mobility as well as ease with ADL's. He refuses pharmaceuticals because of previous problems with opioid addiction.
