Nursing Care Plan
The purpose of this assessment is for students to identify one nursing priority/problem from a chronic care management issue from a case scenario and then design a person-centred nursing intervention plan.
Instructions:
Part 1 - Nursing Care Plan (table)
This care plan (table) aims to assist you to:
a) analyse the information provided in the case scenario,
b) identify one nursing priority/problem (a chronic care management issue), and
c) organise the analysed information using the template provided (ie. table) and briefly outline the goals, actions/interventions and evaluation methods.
Part 2 - Nursing Care Plan (Essay: 1000 words):
Use the principles of person-centred practice to facilitate and formulate a care plan in an essay format. Your care plan should consists of the following components:
a) An introduction: describe the person's situation [approx. 50 words]
b) Identify one nursing priority/problem (a chronic care management issue) and explain why it is a priority (supported by current literature) [approx. 150 words]
c) Establish two goals - ie. describe what you want to happen, desired outcomes and a time frame) [approx. 150 words]
d) Design a course of action (nursing interventions) with rationales - you should consider different alternatives available and use the current literature to support your actions.
e) Outline one to two evaluation methods - ie. describe how you are going to evaluate the effectiveness of the actions [approx. 100 words]
f) A conclusion [approx. 50 words]
Scenario 25-July to 2-August 2021
Overall, Jen made good progress. She had no residual deficits in her right leg. She was able to walk without assistance. However she had mild weakness in her right arm and as a result she was unable to carry out all usual activities. She also had dysarthria (slurred speech). She required long term speech and physio therapies. Jen was very upset about her current situation as she would be unable to continue her teaching career.
Ms Jen Bogart had made a full recovery after a stroke in 2018 (as you have seen on the video). However, on the 24th July 2021, she had another stroke and was admitted to an acute care hospital. She was then transferred to a stroke rehabilitation centre on Monday 2nd August 2021. You are on your workplace experience placement and have been assigned to look after her with a registered nurse over the next two weeks, starting on Tuesday 3rd August 2021.
On the next few pages, you will find Jen's clinical information and relevant clinical charts. You are required to analyse the information provided including the video on previous page* using the nursing care plan table/template (Part 1) and then formulate a care plan in an essay format (Part 2) based on her needs and circumstances, ie. person-centred approach.
Her vital signs when she presented to the emergency department:
• Blood pressure 180/90mmHg
• Heart rate 80 beats per minute, sinus rhythm
• Temperature 37.1 degrees Celsius
• Respiratory rate 10 breaths per minute
• Blood sugar level 5.5mmol/L
• Oxygen saturation was 95% on room air
• Glasgow Coma Scale: 15/15
• Pupils: 3mm, right = left and both reacted to light
• Limb strength (NSW Health Adult Neurological Observation Chart):
• Left arm and leg - normal power (5)
• Right arm and leg - Active movement with gravity eliminated (2)
INR was 1.0 - she wants to get pregnant so she stopped taking warfarin without consulting her medical doctor a few weeks ago.
