Question

Child came to nurse's office after recess stating they felt short of breath. Child was diagnosed with asthma 2 weeks ago. Child appears short of breath; lung sounds have some slight expiratory wheezing; respirations were 22 breaths/min; and oxygen saturation was 93% on room air. Their peak flow meter reading was in the high-yellow zone. Albuterol treatment was administered as prescribed. Respirations improved at 18 breaths/min and oxygen saturation increased to 96% on room air. Child was able to reach the bottom of the green zone on the peak flow meter. Child's parents were notified. Child stated they felt much better and returned to class. Date: 12/13 Time: 1130 Child returned to nurse's office after recess stating they were having difficulty breathing again. Child was only able to reach midway in the yellow zone on their peak flow meter. Inspiratory and expiratory wheezes were auscultated. Respiratory rate was 24 breaths/min and oxygen saturation was 92% on room air. Two puffs of albuterol were administered. Lungs were clear to auscultation and child's respirations and oxygen saturation improved, 16 breaths/min and oxygen saturation was 96% on room air. Child was able to reach very bottom of green zone.
