2. WORKPLACE EVIDENCE
This part of the RPL Kit requires you to provide evidence from the workplace. The evidence could be from your current workplace or one that you have recently been involved in however it must be in the disability care sector whilst performing the role of disability care worker.
You may need to obtain permission from your workplace supervisor to be able to submit the documents requested. Please also ensure that you maintain confidentiality and remove or cross out any identifying names etc. on the documentation.
This documentation is to demonstrate that you contribute research information about compliance and ethical practice responsibilities and then develop and monitor policies and procedures to meet those responsibilities.

Submit evidence to demonstrate when you have delivered services that foster human rights of clients with disability. You must ensure the rights and needs of the person are upheld in the context of person-centeredness. Your evidence must contain the following details:

2. Breach or rights
Submit evidence to demonstrate when you have identified and reported breaches of human rights in your workplace, according to the organisation's policies and procedures.

4. Self determination
Submit evidence to demonstrate your facilitation of choice and self-determination for at least two (2) clients with disabilities. Your evidence must provide details on the following:
The person-centred approach work that you have used to acknowledge the client with disability as their own expert
How you have facilitated person-centred options for action on relevant issues and discuss with the client and/or family and/or carer and/or relevant other
Identify the issues that you have discussed with the following people.
How you have provide assistance to the client with disability to facilitate communication of their personal goals
Identify the goals that they have discussed with you
Describe how you encouraged and empowered the client to make his/her own choices.
Describe also how you have ensured that the client is comfortable with any decisions that are being made on their behalf.
Evidence submitted must be in consultation with the clients, their families/carers and teacher/s.

Staff handbook/memo relating to advocacy and complaints mechanisms
Letter/email of complaint to an appropriate personnel in the organisation
