Part 2 - WORKPLACE EVIDENCE
This part of the RPL Kit requires you to provide evidence from the workplace. The evidence could be from your current workplace or one that you have recently been involved in however it must be in the aged care sector whilst performing the role of aged care support worker.
You may need to obtain permission from your workplace supervisor to be able to submit the documents requested. Please also ensure that you maintain confidentiality and remove or cross out any identifying names etc. on the documentation.
This documentation is to demonstrate that you contribute research information about compliance and ethical practice responsibilities and then develop and monitor policies and procedures to meet those responsibilities.
This unit of competency requires you to provide evidence of completing the following:
Ability to demonstrate essential knowledge required to effectively complete tasks outlined in criteria items, manage tasks and manage contingencies in the context of the work role.
The requirements of this unit also includes:-
Provision of demonstrated evidence that the candidate has: used suitable facilities, equipment and resources, including:
- individualised plans and equipment outlined in the plan
- infection control policies and procedures
- modelling of standard industry operating conditions and contingencies, including involvement of real people when using relevant equipment
1. Job Role
Submit evidence that provides information regarding your job role in the workplace.

2. Basic requirements of the client
Recall an instance when you have provided support to two (2) individuals.
Provide the following information based from each of the person's individualised care plans that you have used:
At least three (3) support needs of the client in the following areas:
- Physical needs
- Psychological needs
- Social needs
- Cultural and spiritual needs
- Sexual needs (if applicable)
At least two (2) basic requirements in the following areas of health:
- mental health
- nutrition and hydration
- exercise
- hygiene
- lifestyle
- oral health
Any of the following documents can be submitted as long as it contains the above-mentioned information required:
- Client's care plans
- Email correspondence
- Meeting minutes
- Person-centred planning tools
- Progress notes
- Client feedback
- Any workplace-related reports/records that provides information on the above-mentioned requirements
3. Implementing the care plan
Submit evidence to support your demonstration of the tasks and activities provided for two (2) separate clients relevant to the following:
- Client rights and complaints procedure
- Procedures followed to assemble the equipment required for client support
- Policies and procedures followed including duty of care when providing support to the client
- Observation from the client when the support activity was provided
Any of the following documents can be submitted as long as it contains the above-mentioned information required:
- Organisational policies and procedures relating to client rights and complaints procedure
- User instruction of equipment assembled
- Organisational policies and procedures for equipment use
- WHS policies and procedures relevant to providing client support
- WHS policies and procedures relevant to maintaining a clean, healthy and safe environment
- Hazard identification form
- Risk register
- Other possible evidence may include:
