Once your Six Sigma analysis is robust, you'll have an FMEA analysis for each PMAP that you've drawn. (They might be in a single spreadsheet, or in many spreadsheets.) Your failure modes will all be violations of a requirement for an output somewhere in the PMAP, and your causes will all be violations of a requirement for an input for the process step that produces that defective output. The effect of the failure mode will be derived from the problem statement in your project charter (either directly from the charter, or indirectly through the cause-and-effect or critical-to-quality diagramming you've done for those problems)

Discuss what you need to do if, in conducting your FMEA analysis, you discover a failure mode, effect, or cause that you feel needs to be included in your analysis, but can't be mapped into the other models according to these rules. Describe how other process models you've developed have to be changed to bring your new FMEA entries into proper alignment. How might this affect your project scope and schedule?
