It is important to identify potential hazards and evaluate the risks involved, noting who might be harmed and how. Risk management is a comprehensive framework that guides risk management and patient safety. To support this effort, it is essential to examine the organization's finances, patient and employee complaints, privacy and confidentiality protocols, infection control procedures, and the number of incidents and medical errors (Hughes & Robinson-Wolf, 2008).Some employees may resist change due to complacency in performing their duties, regardless of whether they are correct. It is the responsibility and accountability of employees to perform their jobs correctly, following Federal guidelines and Occupational Safety and Health Administration (OSHA) guidelines for safety and infection control. Lack of cooperation among employees can also be a hindrance when change is involved.

For example, a nurse who has been doing a procedure one way for years may need to be more cooperative in changing their approach, even if it does not meet Federal or OSHA guidelines (Institute of Medicine, 2000). When I first started as a physician assistant working in home care, I was responsible for providing care based on the nurses' notes and assessments while ensuring their charting met the Plan of Treatment (POT) and Medicare guidelines. If their documentation did not follow these guidelines, I legally could not provide care without an adequate assessment or intake. Some nurses became angry and refused to rewrite their notes, insisting on documenting based on their usual approach rather than following the POT and Medicare guidelines.
