Question

This is a 67 year old Caucasian male with a history of alcoholic cirrhosis who presented to the ER today with 1 day reports of hematochezia, melena, and worsening abdominal, scrotal, and bilateral leg swelling. He states he recently saw his PCP 1 week ago, and had his routine blood work completed, his his doctor informed him his creatinine was slightly elevated at 1.6. His PCP advised him to stop his Lasix and Aldactone. Since then, the patient has noted increased swelling in his legs, abdomen, and scrotum. Today, he noted a large bloody bowel movement which prompted him to come to the ER for evaluation.
