Question

A 70-year-old Caucasian male presents with persistent headaches over the past two weeks that have worsened and begun to interfere with his every-day life. His daughter is present during the exam. She reports that she drove her dad and wanted to be present as she has noticed his memory and ability to concentrate have been "a little off lately". She also states she has noticed her dad experiencing several episodes of what she describes as "spacing out" and "acting off" lately. The patient agrees that he has been struggling to remember simple things or being able to focus lately but does not notice the episodes of "spacing out" that his daughter described. The patient also states he has felt a little clumsier over the past few weeks. He denies any recent illnesses/infections, changes to medication, falls/head injuries, or changes to medications. He denies N/V but does reports occasional episodes of blurred vision with the headaches. He rates his headaches 7/10 on the pain scale and reports taking Tylenol as needed with no relief. He is unsure if anything triggers or worsens the headaches and has found no route of relief at this time. He denies prior headaches of the same.
